I have to read a 350 page book to read which I have to make 50 annotations on due a few days from today and I don't even know the book's title.
Only motivation is self motivation imo. Turn off all electronics and just do it.

Honestly? From what you said,if I were you I'd follow Kodaku advice.
Doing things last minute gives you extra pressure. And you cant sleep easily because if you dont do it you cannot forget: "I have to do that shit.."
So, unless you have another plan you can follow, like defining; "From 6 pm to 9pm I will work and shut all PCs and TVs", do it last minute
