What is this? A voucher for a fitbit? Model is Buyers choice?

EDIT: I see now that you have different prices depending on the model. However how is it that this works? The voucher is paying full price of the fitbit or does it apply a discount for the fitbit when used?

After someone is issued a Fitbit replacement they are sent a link to one of these.
Fill it out and you get sent a Fitbit to your address.
