Try turning your volume up, if that doesn't work, download a loud music file and replace the Ghost breath sound file. May be a bit confusing in game, but you'll hear them.

but this is the default idea:
GAMMA 100
SOUND EFFECTS 90-100 cuz when u hear gunshots its disturbing
USE HEADPHONES
USE GOOD GUN! I USE XM ADVANCED(also pistol if u run out of primary ammo)
PLAY FAIR! cuz ghosts cant hack! its annoying the fact that 70% has fhakin axe and shovel..
OR U CAN USE some cool hacks out of here
