The game is based off of interacting with NPCs and talking to them and doing quests/missions and joining factions.

It's a game, it's controlable wether it's First person view, or Third person view, you can go on quests, level up, join groups, etc. It's fun. Google it for more information

I know that but I've seen many videos where you go off into the woods by yourself and like going into those scary caves..... I'm thinkin more about games like GTA or other sandbox games.

I'm the head of the Dark brotherhood but I just joined the Thieves Guild. Do I just steal stuff, sell the stuff to the guy, and then do missions?

I got bored with it when I got head of both "bad" factions. So I added in some cheat codes for the best armor/weapons.
