You open Extreme Injector -> Add DLL, choose the one you downloaded in the .rar archive (MW3SteamHook.dll) -> Make sure MW3 is open and choose the iw5mp.exe process -> Inject.
A menu should appear in-game where you can choose the ESP settings.
