I've used it, haven't been banned.
Open the unlocker first, then open Call of Duty: Modern Warfare 3. This removes the error that people are getting if you have MW3 open before the unlocker.

Go to your barracks [VAC isn't enabled here], ALT-TAB to the unlocker and proceed to modify your stats etc.

Exit the unlocker and then play a game to save changes. EXITING THE UNLOCKER BEFORE PLAYING IS IMPORTANT!

Not all titles and emblems are unlocked, weapon emblems need to be unlocked via gameplay - 5 kills with each weapon approx, try and
get a headshot, makes things easier :P
I've noticed the AS50 kills count as 1 towards your record.

Emblems that require 1000 kills with a killstreak are locked, same with CoD - ELiTE emblems.

It took me about 1 hour to get all my guns gold - 1 Kill = Gold but it takes a few kills to achieve maximum mastery.

All in all - amazing unlocker - Wouldn't mind an update to complete everything but hey, I'm Prestige 20, everything gold.
