I think some people can modify it without getting caught but it is still very risky and if you don't get caught in the first place, you will in the future.

Nope you can't turn on sv cheats unless you have rcon password of the server.

Incorrect, VAC doesn't check for sv_cheats. They'll check for the memory block CVARs are in for memory edits though.
The source engine used to have a bug where you could incrementvar on it, people could enable sv_cheats and no VAC ban was ever given for it since it was a built-in engine bug.
