To put it simply when you get reported a lot. Other players review the the game you were reported in and decide whether you cheated or not. That's it. If you cheated then you will be banned. If you didn't then you won't. However false positive over watches happen and when you do get judged unfairly you can go to steam support and get your ban removed.
