coinbase is always the best, always shows you amount of btc you have in USD, fees are very small, very quick and almost never down, used over $10,000 on coinbase. no issues at all!

no, only if you buy bitcoins, i've received transactions as big as $400 before, you just need a ID and password, and a phone, they use your phone to send you text to make sure it's you, you don't need to put bank, creditcard, ssn, etc.!
EDIT: added u on skype for a vpn acc

i personally never use any web - based btc wallet (i'm just afraid their sites is down when needed like blockchain yesterday)
also i read that some users is banned from the sites without any reason and they lost their btc there
for this past year i'm using multibit HD (it's a wallet application can be accessed both in windows and mac)
so it doesn't require any ID or something, it can also be automatically backed up to clouds (i'm using dropb*x)
the cons is there is no mobile app for it, while most of the web based wallet has
so ya, just personal thoughts, i actually want to move to online web based wallet but idk maybe i'm just too afraid

Coinbase and blockchain are probably the most trusted ones, but you can use exchange services like cex io (which I think have the best security out there), bitstamp or btc-e as wallets without a problem

I'm using cryptopay atm. It is easy to use and has 4 different wallets. Pounds, USD, Bitcoins and something else I don't really remember.

I currently use Coinbase.
In most of the website you do not need any real information to make an account.
You may receive and send as much money you want (almost)
You only need ID to BUY/SELL Bitcoin or to withdraw/deposit in a bank account.
