A Serbian Film is apparently really disgusting. I haven't seen it myself but my friend and a review I read really suggests that it's really fucked up.
EDIT - Seems people have already mentioned this movie.Let me tell you one thing:I have seen the sickest horror/exploitation movies ever made including "Cannibal Holocaust","Mu Zan E","Forced Entry","The Taming of Rebecca","August Underground" series,"Angst","In a Glass Cage","First Transmission","Despair","Aftermath","Man Behind the Sun","Lolita Vibrator Torture","Martyrs" or "The Untold Story",but some ideas presented in "A Serbian Film" left me speechless.

And yeah, he's got shitfucktons of awesome movies, though for the most part Antichrist is probably the one closest to what most people perceive to be a "normal" movie. Goddamn Hollywood. I actually liked Melancholia because of the whole Wagner + German Romanticism degeneration or w/e, but I would hardly label it as "fucked up", it's actually closer to his earlier works when he toyed around with love and shit
His movies are good, I just don't like them... Antichrist was just a sick idea and a lot of religious people got kinda upset.
All in all he is cool and i respect him though i don't like his movies.
