The second one is weird, it happen when you try to connect on a server where you are banned -without- the unbanner first, and try to use it after
Your unbanner can't bypass the ban after this, and it lasts few days, no matter how much time you close your game/computer/router, the ban will lasts few days (2-4 generally) I don't know why, but it works after

INJECT ONLY IN MAIN MENU. (WAIT 5 SECONDS PRIOR TO INJECTION)

For TeknoMW3, just join a server for it to auto-activate as with all versions.
For IW5M, on the "Connecting..." screen, press Shift+Enter within 5 seconds. Your name and GUID automatically resets to the original within 20 seconds. This does not occur with TeknoMW3. I have verified this is patched on IW5M. Do not complain if, in your ignorance, you use this and get banned.

Exactly.
When you say something is patched, it means that you can't use it anymore, and you have a lot of change of being banned
If detected, you can use it, but you will be banned too
