You need to get a new hard drive. Replace the hard drive(s) you currently have with the new one and don't use the old ones. Don't try to log in with the banned accounts either or the new HD will also...

You literally have to buy a new graphics card, and the participating retailers will send you a code with purchase. If you already have an NVIDIA graphics card, it doesn't count.
