Format:

Type Of Mod: Texture, Cross Mod 

Weapons Or Map To Be Edited: e.g. L96A1 or Snow Valley

Which REZ File It Is located In: e.g. WORLDS.REZ (If Your Not Sure Leave It Out)

Name Of File To Be Edited: e.g. L96A1.dtx (If Your Not Sure Leave It Out)

Color ( If You Want A Cetain Color On The GuN): e.g. PINK!

Theme (If Its A Texture Mod): e.g. Abstract

Image (If You Want A Image Placed On Your Gun): e.g. www.tinypic.com/example

Text (If You Want Anything Written On The Mod): e.g. "sendi11 is f**king awsome"

Name Of Modder (If You Want Someone Specific To Make Your Mod): e.g. sendi11

For Cross Mods:

Weapon To Change: e.g. L96A1

Weapon Wanted: e.g. L96A1 Arctic Wolf

Sound Changed: Yes or No

I think telling the mod creator where the file is they want modified is a bit excessive. People making requests probably can't open rez files.
