they go on the site and login. they go to the change acc details and then they go ingame and login to make the person change their pass and the SQ usually comes up.

A hacker isnt able to just "hack" your account.
They normally get your information via a keylogger that reads what you type and sends it to the creator of the keyloger.
and then there are also Phishers that are normally in a form of ZP/GP gens that ask for your account info in return for unlimited ZP/GP ectDo not go gentle into that good night,
