It's Happening From Along Time I Guess, But I Just Knew Yesterday..
It Goes To 112c-116c (Temp.) After Minutes From Opening The Pc, I Cleaned The Whole Cpu And The Graphic Card Fan And Everything And It Went Down To 60c For Some Hours And Got Back For Getting Hot As Hell Again !

make sure the fan is clear of dust. I just cleaned my computer and removed a shit load of dust.
Dust is an obvious issue. Making sure you have proper position of intake and exhaust fans, believe it or not, have a huge impact on GPU temperatures. You just have to find the right setup to help mingle with your components.

You could use nvidiaInspector to lower your temperature. Mine goes around 35-38 sometimes when idle but this tool makes my GPU go down to 0.875v and 30degrees when surfing flash sites and stuff. Thank me later and if you need help how to use the tool just give me a shout
First of all, people recommending watercooling to this guy have no idea what they're doing.
You're in dire need of a new videocard alltogether, and GPU waterblocks are NOT universal, so if you'd really want watercooling you'd have to get a new card and then a waterblock for that (plus the entire loop system, so really, don't even consider it, a good brand videocard will cool well enough).
If you really want to keep using it, re-new the thermal paste on the GPU and clean the entire internals for dust. This means removing the cooler (which is needed to re-apply thermal compound anyways.)
This however, is a slightly more advanced step and I'd just recommend a new videocard.
