Hundreds of new enemies.
Completely changed gameplay.
Much better graphics/weather/colors. (Hell, my FO3 rivals in Crysis in visual quality.)

I wouldn't have 2.2k hours into it without mods.

I tried using Nexus to mod and it was annoying so I stopped for a bit. But FO3 is amazing and the modders do a hell of a job.

I dunno, it just doesn't feel like a game that should be modded for me even though there's thousands of mods out there.
I haven't even modded skyrim...
I think the only game I've ever heavily modified was gta IV and that game doesn't even support mods
