I often read old threads and have seen people with high post counts and old join dates perma banned. I would just like to avoid this so I'm wondering what typically happens that you would even perma ban veterans (not that anyone is exempt)? thanks

That pretty much explains it. As well as repeat offenses of those rules can get you banned permanently. It is also important to know, that it is not up to the member's POV if a rule is broken and how severe the punishment should be, but to the issuing staff member. Usually it is not an issue, but sometimes some members believe they can flame, infect, or otherwise hinder other members or staff and that they should only receive the minimum (if any) punishment. They're often proven wrong.
Hope that helps.
