Drowning/Falling from a plane.

when u go parachuting and find out parachute wont come out
Being skinned alive while being spoken to in a dirty manner.

Perhaps gettin your fingernails torn off leading into skinning of only your fingers repeated punches to your face (why not) constant fire burning your feet a knife scraping away at your knee caps having your ears cut off by a rusty spoon having a non-greased shotgun shoved up your ass (as opposed to a greasy one) being scalped by a tomahawk put some salt/citrus fruit into it all and then as a finisher choking on piss and shit after that! I'd say that's a pretty terrible way to die granted its all going on at the same time.
