FSOD System Exception Error

The server was working very well before I started making major changes,
I added in dungeons to the spawn that have bosses spawning inside (So its basically an infinite dungeon)
Anyways, when a death occurs inside the dungeon or after two clients have loaded and left the dungeon becomes unjoinable again
I'm pretty sure the error is shown in my console screenshots, if anyone has suggestions please let me know!

I would recommend 1) Checking your Sql database, 2) Making sure the limit to the dungeon or server is more and 3) Check the files for the dungeon, try and look at another dungeon that works and see whats wrong. It could be something with the portal key.
