FAQ - As of September 19:
- The tool still works, yes (and it should be working for quite a while since it accesses the client API; No reason for riot to change it atm)
- You find a link to interesting/hidden icon ids in the program

If your icon resets this might resolve the issue:
- Right click the config folder inside your League of Legends folder (C:\Riot Games\League of legends)
- Hit open properties
- Unselect read-only and apply it recursively to the entire configuration folder

The icon is changed when on the client, but when loading a game or in-game my icon is just black, is that normal?

Yep, ingame the icons are not available. (I have no idea why)

The ban chances for this aren't unlikely no, anything that edits or manipulates or the usage of any TPS, is a chance of ban, and non the less a perma ban btw.
The usage of anything outside league to change league in any way what so ever, even if you just changed the sprite to remove the armor from a certain champion, lets say Garen, or katarina, by altering anything, you will receive a perma ban. However it can be appealed and if you're lucky you can get unbanned, but they have been very clear about this
