if you're using a middleman, you give the NX code or cash to him. the MM will mail a cod for in-game junk item to buyer. after he has the gold from the cod and cash/code from you, (and verifies the NX code is legit if applicable,) he will give the code/cash to the other guy and you will cod his char with a junk item for the amount agreed on.
The gold seller rates are about $26/100M, but some of the MPGH people are selling as low as $18/100M right now.
