What about trying every feature and see wheres the problem exatcly?
Also, thank CA that they built in this messagebox. It shows you where exactly it crashes.
Why not log it and copy the address here?
This should be very easy to fix at all.
I finally got a menu hack to work via updating it. (Using CA Base D3D Menu Selection 4). My issue is a bit difficult, to explain, but I'll try my best to do so.
When I first got the hack to work, I only had 3 Features: Show FPS, NX Chams, and Move Menu. With just these 3, the hack worked fine. (No lag/crashes) But now, I added a few features:
NX Chams
No Recoil
No Spread
Fly
Speed
No Fog
Unl. Pickup Range
Super Jump
Rapid Fire
Nametags
Weapon Range
Show FPS
and CrossHair.
Upon adding these features, I now get an error when CA loads (guy is running on bot right corner)
Something like "The instruction 0xblahblah referenced at 0x0000000(has diff addresses at times) can not be read. Press Ok to terminate"... Etc.
I began to remove 1 at a time to see what was causing it.
Apparently,
Rapid fire,
Nametags,
and fly
were the issue.
After removing those features, I started CA, and my menu was visible. however, the lag is crazy! I get 10-14 FPS using my hack now. With this lag, I decided to go in a random match to test what would work. I joined a random room, only to receive the same error (with different addresses). "The instruction 0xblahblah......", which results in a crash.
So these are the errors I basically receive:
All features = startup, receive error, crash.
Most features = starts up, lags badly, crashes when a game is loading. (ex: loading at Junk Flea)
Some features (3) = no issues
I am just stumped with this issue. Can anyone tell me, or at least give me a hint, at what I am doing wrong?

Progress with my game - "Disbanded"
- Fixed FPS lag on spawning entities due to the ent_preload buffer!
- Edit the AI code to get some better pathfinding
- Fixed the view bug within the sniper scope view. The mirror entity is invisible now!
- Added a new silencer for ALL weapons. Also fixed the rotation bugs
- Added a ton of new weapons and the choice to choose a silencer for every weapon
- Created a simple AntiCheat, noobs will cry like hell xD
- The name will be Disbanded, the alpha starts on the 18th august 2014

I will put in 1 feature at a time :P(Rapidfire is one of the problems though) & show you the error I get.
How do I prevent my hack from lagging up my frame rate? o_o

Thank you @Flengo! I fixed the lag, but I still get the error message crash.
Rapidfire is currently the only feature that is causing it.

Edit: After commenting out Nametags, and not enabling Rapidfire, my hack works fine. :O
