Are you using MPGH's Public AVA Hack? If you are, the menu for the hack in-game isn't insert. Try pressing Delete key.

EDIT: Also if it doesn't work, try using this injector, named: PerX.

PerX doesn't do it anymore for A.V.A

Sorry, but PerX still works for me on A.V.A, i've been using it everyday.

Also, the program with him, is that he pressed Insert instead of Delete.
The .DLL file he posted in that picture is named AVAPUB.dll, which is the MPGH A.V.A Public Hack .DLL name. If i'm not wrong, you have to press Delete key and not Insert key.
