Start the game and wait till you start loading the level and then alt tab and start the hack, make sure your in windowed borderless.

Why my ESP didnt work, but another feather work normally
The ESP is really buggy and is not working normally as intended, if enabled it will drop significant amount of FPS, just use the Minimap one, at least that one is working better, it reveals every single enemies in the map. Also there are times that the options you selected will wear off after respawning or loading into a new game, just re enable them at that point.
Things to consider before using this hack.
1. The hack uses different floating point code which is why it is required to have your game set to either Windowed or Borderless (As long as its not fullscreen)
2. Open the game first before opening the hack (the hack wont open if you do it the other way around), has to be opened before joining any server, reason for that is Punkbuster gets launched every time you join in one, so if you opened the hack after you joined a server, chances of PB kicking you out might come.
3. Just to make sure, use the Test Server to try out the options.
4. As I mentioned on the top paragraphs, ESP dont really work well and will drain almost a lot of FPS, I havent tried the aimbot but who uses it anyway (i guess some raging kid who just wants to kill that one guy). The minimap option is the best way to go instead of ESP, I dont know if the DrawD option has something to do with it but just make sure to set it to max, my guess is its the amount of detection by meters on the map.
5. Another thing mentioned above is that the effects of the hack sometimes wears off when respawning or joining a new server, just re-enable them at that point (you dont have to worry about PB detecting you since the hack was injected before joining a server)

Are you using the .exe one or the one that isn't compiled and do you have autohotkey installed?
Its still undetected for sure? How long has this been out exactly?
For anyone experiencing the issue where the menu appears out of the screen, this could be due to scaling settings for your display. I had mine set to 150%, but when reduced to 100% it appeared normally. Would like some updates if anyone is banned, I've used for 3-4 days now (no recoil and spread only) and have not had any issues or suspicions.
When i used it it doesn't open all the way the menu I only get the no recoil and spread it use to work with all options, maybe something on my end but it does work and FYI if you get kick by punkbuster doesn't mean because of the cheat Punkbuster is very unstable. But download the cheat play with it fun 1
