All the functions are there but some thoughts you could use if you were to create another one would be to either create a unique or professional theme to it which could probably include

- Evenly spaced button
- Not a lot of free space to look at (unless your intending to do it for an effect)
- Form name
- Larger (longer) text boxes.

Besides that it looks excellent! good work.

yahh and make ur labels transparent ... put a name for u form ... get a icon .... just go google and search a picture then go to google again and type Ico Convertor ... it will convert icon .. ? by the way nice job .... ? it is easy
