if you are not getting the email from insurance company, then do it again and this tme don't close in the last page go ahead and fill with CC info until it says invalid credit card then you will get the mail.

If you not seeing "Report Problem" then you should wait 20 to 30 mins and for the E-mail of insurance company, you have to wait most probably 12 hours.
I have given proofs by screenshots so no complaints.
