Why the most of macro required you to change 'fire' to middle mouse button ? Can't you use the left click ?

You know I was wondering something like this, Ive been using this macro recently. But most of the time I have been using the other macro (cant think of who made it) but so I set the fire to MM but MM doesnt activate the auto click, it still shoots but the auto click is still my left click?? But then when I close out the macro, left click doesnt work at all?? Can someone help me out with this? Is that how its supposed to work???
