How do you use FdHackV2?

I've tried over and over. Do I put the hook.dll into my CoD4 directory or something? I've got past it just terminating, but now it continuously tries to inject and fails. Any help would be much appreciated. If it helps any, I'm running Windows 7 x64bit.
