1.
WAR MACHINE
You have killed the War Monger Zombie Mode boss 100 times on normal difficulty.

Does it mean that I need to do it by my self? Or in team play?

2.
LEAD THE INVASION
You have played in 100 Hero Mode X games where the Mutant side has won

How I can check this up?

3.
MY RIBBONS ARE MADE OF CRYSTAL
You have collected 20 or more Ribbons
M4A1 - Custom Crystal

Is it permanent?

War machine ~ Team or solo it as long as you actually defeat it and get to the ending screen.
Lead the Invasion ~ Can't(not to sure about this one)
Ribbons are made of out crystal ~ Yes.

To answer your questions and to add on to what Blue Turd said,

1. It doesn't matter, as long as you get to the end and get to see your final score

didn't die, etc.

2. What I did was continually play HMX until I received the ribbons, only took me a day or two

3. Yes, the M4 Custom Crystal is permanent, and it's amazing.
