Make sure you have a mods folder inside the AlterIW directory. Inside the mods folder, there should be another folder for each mod. Let's say that I had a folder named mpgh in there and inside the mpgh folder was mpgh.iwd. I type this to load that mod:
fs_game mods/mpgh; vid_restart
