No you're over thinking this and obviously didn't try my simple solution.
or push f7
you cannot just use the debugger and run it, it's a dll.

I'm pretty sure sunkist is right. Once you build your dll use an injector to inject it into the game.
How does one "start" a DLL file exactly?

By loading it to memory via a system call like loadlibrary?

And LoadLibrary isn't a system call.

Eh? Yes it is, requesting windows to load a dll to memory and call it's main function?
Requesting system to do something = System call.
I hope that you're not confusing it with system COMMAND.

Except it isn't requesting the system to transfer control from ring3 to a lower ring and back.
That's the definition of a system call.
Loading a DLL and calling the entry point takes place inside Kernel32.dll and KernelBase.dll which is why manual mapping of DLLs is possible without performing any actual ntdll functions. ( Aside from allocating the page region and setting the page access rights )
If you're not transferring control to the kernel via the call gate or KiFastSystemCall/interrupt vectors, it isn't an actual system call since it's actually done in userland.
So, eh, no.
The real question here is, @Maggkollin were you able to solve it with the suggestions provided?
You compiled the dll successfully but you can't start it because it's not an executable.
